Anytime a member(s) of a United Methodist Church decides to move their membership to a new church, a certificate of transfer must be completed as required by The Book of Discipline of The United Methodist Church. This Official Certificates makes this task easy for both the sending church and the receiving church.
Upon on request for a transfer of membership, a certificate is completed and distributed. Each certificate is perforate between what the sending church keeps for their records and what is divided up for distribution. There are a total of four sections; each one with a unique purpose. The first three sections are Notice of Transfer of Membership and the last section is an Acknowledgement of Transfer of Membership.
Section 1: The sending church fills out this portion of the certificate and it remains in the booklet.
Section 2: The sending church completes this portion and sends it to the transferring member(s).
Section 3: The sending church completes this portion and sends it to the receiving church. (Only this section has space on the reverse side for listing all children transferring with space for date of baptism if applicable.)
Section 4: The receiving church fills out this portion and returns it to the sending church.
